I have written couple of times now on feedback and its importance for organisational performance, what I would like to touch on now is the whole issue of performance management at an organisational level.
What follows does not come from a study of performance management systems but from the practical, real-world experience I have had with clients and the challenges they have faced up to.

'Yeah, yeah... I've heard it a thousand times, I know listening is important and I don't need to be told again'
The problem with having heard something time and time again is that we tend to develop mechanisms which filter out what in our minds, is either 'noise' or something already known.
Anything that leads to a great outcome is either the result of chance or effort. Chance is something that we can't do that much about so we are left with simple concept of working at it.
